Everyone sees the world differently. Sherlock has a very set view of himself, but everyone else only sees the snippets he allows them to see.
Yes. ;P Glad you noticed John was the only one who really honest to God accomodated Sherlock without expecting/wanting anything in return.
Moriarty's scene was one of the most fun to write. I love writing insanity pieces. :DDD
